If you are a California resident, under the California Consumer Privacy Act of 2018 (“CCPA”), you have the right to request to know about the personal information we hold about you and the right to request that we delete the personal information we collected from you. Please note that any request to delete personal information will be subject to verification and review for exceptions to granting the request as provided in our Privacy Policy and under the CCPA.

You may exercise your right under this Form twice a year, free of charge. For any requests in excess of two per year, we reserve the right to charge a reasonable fee associated with the actions you are requesting. Please note, the CCPA prohibits us from disclosing your Social Security number, driver’s license number or other government-issued identification number, financial account number, any health insurance or medical identification number, any account password or security questions or answers in response to a request to know.
